- Enter the name of the nominee, D.O.B, Relation which are mandatory
- If the relation ship with the nominee is not in the list select other and enter
the relation in the Others relationship field

Step 3:(Click to View Image)
- Enter the permanent address details
- Select the country from the list, corresponding state will be list in next combo,
then select the state corresponding city will be listed, then select the city which
are mandatory.
- Any one of the Phone is mandatory.

Step 4:(Click to View Image)
- If the permanent is same as the contact address then there is no need of entering
the details if not select the radio button Not as same address

Step 5:(Click to View Image)
- If all the details are entered click the continue buttonWhich takes
you to the view page

Step 6:(Click to View Image)
- The details we entered can be viewed here and edited if required by clicking the
Change details link
- If you find all the details to be fine then enter the code shown in the figure in
the below text box and click the Complete Registration button
- On clicking the button the Thanking page is displayed as a mark of registration
